ESA Facility Waivers

Below are the required waivers that need to be submitted once per season (September 1 annually marks the start of a new season).  Every person accessing the facility in any capacity other than as a spectator is expected to submit these documents.

Adult Waiver

Adult Waiver To be signed by individuals who are 18 or older as of the date of signing and submitting the waiver, regardless of the age group(s) of the leagues/divisions that they are playing in.

Youth Assumption of Risk

Minor (Under 18) Assumption of Risk to be signed by the parent or legal guardian of individuals who are under the age of 18 as of the date of signing and submitting the form, regardless of the age group(s) of the leagues/divisions that they are playing in.
